*  All household members, have not been diagnosed or have been exposed with COVID-19 within the past 30 days.

*  I understand that Divine Harmony Healing and G Fontana Salon Studio cannot be held liable for any exposure to the 

     COVID-19 virus as I have decided to come here on my own free will and will not hold them responsible should I contract COVID-19.

    I understand that:

*  Physical distancing of 6 ft. is not possible while receiving energy healing services.

*  A mask must be worn while laying face up during treatment and when entering and leaving the treatment room.

*  Hands must be washed and sanitized my hands before entering the treatment Room. 
*  In the event of coughing or sneezing hand must be immediately sanitized!
*  I may not bring children to your healing session.

*  Our establishment has and will continue to be sanitized and safe according to the CDC guidelines.
 

*  I understand that we will do everything possible to minimize the spread of COVID-19, by sanitizing the table and room and will be          wearing a mask during your treatment.
